How to Embed a Google Form in your Google Site Contact Page?

Using Google Forms to gather responses for a quiz, survey, or questionnaire? Once your form is ready, you can simply share the link—but if you really want to make it easy for people to access, adding it directly to your website is a great idea. If you’re using Google Sites, embedding your form as a ‘Contact Us’ page is super straightforward. Here’s how you can add it step by step.

Step #1: Create a Google Form

You may use it as a contact form / feedback form.

Step #2: Click the Send button

Step #3: Click the Embed icon (< >)

Step #4: Copy the Embed HTML code

Step #5: Create a Contact Us page

Step #6: Go to ‘Pages’ tab and select ‘Contact Us’

Step #7: Go to ‘Insert’ tab and click ‘<> Embed’

Step #8: Paste the Embed HTML code in the Embed code field

Step #9: Click ‘Next’

Step #10: Click ’Insert’

Step #11: The Google form will show up on the Contact Us page.

You may adjust its height as per the requirement by dragging the margins (blue dots).

Step #12: Click the Publish button

And , this is how the Contact Us page looks after publishing.
