On June 12, 2025, Air India Flight AI-171, a Boeing 787–8 Dreamliner en route from Ahmedabad to London, crashed minutes after takeoff, killing 241 people on board and 19 on the ground. Only one person survived. The catastrophic crash triggered a global response and a high-priority investigation led by India’s Aircraft Accident Investigation Bureau (AAIB).

The preliminary crash report, released on July 11, 2025, delivered shocking findings. Notably, the aircraft had no mechanical or technical faults; its engines and systems were fully functional before takeoff. However, investigators discovered that both manual fuel cutoff switches were activated shortly after liftoff, causing complete engine shutdown and a fatal descent.

The Cockpit Voice Recorder (CVR) captured a chilling exchange between the pilots:
“Why did you shut off the fuel?”
“I didn’t.”
This dialogue indicates confusion and suggests the action may have been accidental, intentional, or the result of an unknown cockpit event.

Air India, now under Tata Group’s ownership, is undergoing restructuring and faces renewed scrutiny over cockpit protocols, pilot training, and safety measures. The airline has urged restraint in drawing conclusions until the final report is released.

Regulators like India’s DGCA have already ordered inspections of fuel cutoff systems on all Boeing 787s, while international bodies like South Korea’s aviation authority are following suit. The FAA has not yet issued a global directive, pending further analysis.

Unanswered questions persist:

• Was it error, sabotage, or something more sinister?
